Release

I/We hereby agree to save and hold harmless East Coast Believers Church and East Coast Dream Center and any oftheir departments, agencies, officers, or employees, all of whom while working within their respective authority, from all cost, injury and damage incurred by any of the above, and from any other injury or damage to any person or property whatsoever, any of which caused by an activity, condition or event arising out of the performance, preparation for performance or nonperformance of any provision of this agreement by any entities providing said assistance, from any liability for the assistance received. The above cost, injury, damage or other injury or damage incurred to or by any of the above shall include, in the event of an action, court costs, expenses of litigation and reasonable attorney’s fees. This Release and Hold Harmless clause is not intended to indemnify against any cost or damage, or portion thereof, caused by East Coast Believers Church and East Coast Dream Center, and any of its departments, agencies, officers, employees, or relationships.

Children

I release the staff of East Coast Believers Church and East Coast Dream Center from any liabilities, claims and demands arising out of injury or accident incurred by my child(ren) while they are participating in any event while at or on the grounds of East Coast Believers Dream Center. I acknowledge and understand that my child(ren) may be unsupervised while on the premises. In the event of an accident and I cannot be reached, I further authorize the executive staff of East Coast Believers Church and East Coast Dream Center to obtain any medical treatment they may deem necessary for my child’s well-being.

Photo/Media Release

I hereby authorize the use of photographs, motion pictures, videos, and electronic sound recordings for East Coast Believers Church and East Coast Dream Center now or in the future, whether that use is known to me or unknown. I authorize the use of any such photographic or electronic productions of me and my child(ren) for any purpose, including but not limited to educational and other public media as may be deemed appropriate by East Coast Believers Church and East Coast Dream Center.

Booking notice: By booking a class or event with East Coast Dream Center, you acknowledge that you have read and agree to this Waiver & Release, Children’s Release, and Photo/Media Release, on behalf of yourself and any minor children attending with you.
